(ITC) dba FBX Internet and the company submitting this form to enable them to become a Retail Marketing Partner (RMP) of ITC for the promotion and sale of FBX Internet access. ITC will provide information and materials for the RMP to successfully promote FBX to its customers. These materials may include, but are not limited to: signage, point of purchase displays, sales brochures and start-up FBX service discs. ITC agrees to compensate the Retail Marketing Partner for each new, first-time subscriber to FBX under the terms outlined in the RMP program. During the sign-up process, the customer (or RMP) must properly enter the RMP’s unique sign-up code, which is assigned by ITC after submission of this form.
